经皮侧入路腰椎间盘切除术的临床应用

摘要 目的 评价经皮侧入路腰椎间盘切除术（Percutaneous lateral lumbar discectomy , PLLD)的手术疗效和安全性。方法 2006 年4月～2010 年5月对163例腰椎间盘突出症患者行经皮侧入路腰椎间盘切除术。应用VAS评分及JOA评分评价手术疗效。结果 163 例患者均成功完成经皮侧入路腰椎间盘切除术，手术成功率100%，无血管、肠管及神经损伤等并发症。VAS评分由术前（6.75±2.32）降到术后（3.10±1.34），P<0.05。JOA评分由术前（11.84±2.42）提高到术后（25.36±3.24），P<0.05。术后随访1～5年，优良率达94.5 %。结论 经皮侧入路腰椎间盘切除术手术安全性高，创伤小，疗效好，适用于纤维环受损的腰椎间盘突出。

Clinical application of percutaneous lateral lumbar discectomy

 Abstract: Objective To evaluate the safety and efficacy of Percutaneous lateral lumbar discectomy （PLLD）．Methods 163 patients with lower lumbar vertebra diseases who underwent the PLLD were reviewed retrospectively from April 2006 to May 2010。The clinical outcome was evaluated by VAS scoring and JOA scoring。Results  All of 163 cases were followed up for 1 Year to 5 years, All cases had been successfully operated without any complication of blood vessel intestinal canal and neural lesion，and the successful rate of operation was 100%..The VAS score was 6.75±2.32 of preoperation and 310±1.34 of postoperation, The JOA score was 11.84±2.42 of preoperation and 25.36±3.24 of postoperation，which had good to excellent rate of 94.5%. There was a significant difference in the pre- operation and post- operation scores ( P<0.05). Conclusions　 PLLD is a valuable method to the treatment of L3-5 Severe intervertebral disc herniation with high safety, less injury and good outcome.
